Introduction
The One World Sight Alliance ("OWSA") is an interactive application website maintained by Lions World Vision Institute for the placement and distribution of ocular tissue to the ophthalmic global community. Please feel free to utilize all features on OWSA. At the same time, please remember that your access to OWSA is subject to the following terms, conditions, and notices (the "Terms"), as well as all applicable laws. By accessing and using OWSA, you accept the Terms without limitation or qualification. LWVI may at any time revise these Terms by updating this posting. You are bound by such revisions and should therefore visit this page to review the current Terms from time to time. LWVI reserves the right, without notice and for any reason, to remove any content on this site, and to deny access of any user or users to all or any part of this site.

Downloading
You may download material displayed on OWSA for appropriate businessuse only. You must retain all copyright and other proprietary notices on downloaded and/or copied material. Any copy of material from OWSA you make and distribute for any purpose must also include these Terms. You may not distribute, modify, transmit, use, or reuse any of the contents of OWSA for any public or commercial purpose without the written consent of LWVI.

You should assume everything on OWSA is copyrighted. Images displayed on OWSA are either the property of, or used with permission by, LWVI. The trademarks, logos, and service marks (the "Trademarks") displayed on OWSA are the property of LWVI and other parties and may be registered and unregistered Trademarks of LWVI and others. Nothing contained on OWSA should be construed as granting by implication, estoppel, or otherwise, any license or right to use any Trademark displayed on OWSA without the written permission of LWVI or such third party that may own the Trademark. Misuse of any Trademarks, or any other content, displayed on OWSA is prohibited. LWVI reserves the right to enforce its intellectual property rights through civil and criminal proceedings.

No Warranties
Except for any express warranties stated on OWSA, if any, the information and services provided or referenced on this site are provided "as is" and with all faults, and the entire risk as to satisfactory quality, performance, accuracy and effort is with the user. While LWVI uses reasonable efforts to include accurate and up-to-date information on the OWSA, LWVI makes no warranties or representations as to its accuracy. LWVI may periodically add, change, or improve any of the information, products, services, programs, and technology described on OWSA without notice. LWVI assumes no liability or responsibility for any errors or omissions in the content of OWSA. In fact, your use of OWSA is at your own risk. Neither LWVI nor any other party involved in creating, producing, or delivering OWSA is liable for any direct, indirect, incidental, consequential, or punitive damages arising out of your access to, or use of, OWSA. For example, LWVI assumes no responsibility for, and will not be liable for, any damages from any viruses which may affect your computer equipment or other property on account of your access to, use of, or downloading from, OWSA.

Availability of Product
LWVI operates and controls OWSA from its headquarters in Tampa, Florida, USA. We do not in any way imply that the materials on the site or the products we offer are available as they may become unavailable or obsolete. LWVI offers ocular tissue to its members in a fair and equitable manner.

Product Traceability
In order to accomplish product traceability, LWVI requires members to complete recipient follow-up forms on OWSA for each ocular tissue received for transplant. Failure to complete Recipient Forms, Post-Operative Reports, Adverse Reaction Reports (if applicable) and Tissue Disposal Forms (if applicable) on a timely manner may result in temporary account suspension of OWSA.

Payment Policy
The terms of payments are 30 days from the date of Invoice unless otherwise changed by LWVI. There will be no refunds or credits given for ocular tissue once it departs LWVI, regardless if tissue is lost, destroyed or damaged. Payments via credit card can be made on your member account on the "Make A Payment" button. Wire transfer details will be noted on each invoice. You may determine the method of payment that is most convenient. If account is overdue, OWSA reserves the right to assess late charges to your account.

Access to Member Areas
Access to and use of member-only, password-protected areas of OWSA are restricted to members of LWVI and authorized users only. Do not share login credentials with third parties or disclose the contents there within. Unauthorized individuals attempting to access these areas of OWSA may be subject to prosecution. Typed in signatures are valid and are considered electronic signatures. Access to your account may be temporarily suspended if your financial account is overdue or failure to complete recipient follow-up forms in a timely manner. Access to your account may become temporarily inactive due to 6 months of inactivity. Your account may be locked due to more than 4 consecutive login failures. Your account may be permanently suspended for misuse of OWSA, any attempt to contact Member Eye Banks, or sharing proprietary information with third parties. There is no charge or fees associated with membership. LWVI and member may cancel membership at any time.

No Liability
THE INFORMATION, PRODUCTS, AND SERVICES PUBLISHED ON OWSA MAY INCLUDE INACCURACIES OR TYPOGRAPHICAL ERRORS. CHANGES ARE PERIODICALLY ADDED TO THE INFORMATION HEREIN. LWVI AND/OR ITS RESPECTIVE SUPPLIERS MAY MAKE IMPROVEMENTS AND/OR CHANGES IN OWSA AT ANY TIME. LWVI AND/OR ITS RESPECTIVE SUPPLIERS MAKE NO REPRESENTATIONS ABOUT THE SUITABILITY OF THE INFORMATION, PRODUCTS, AND SERVICES CONTAINED ON OWSA FOR ANY PURPOSE. IN NO EVENT SHALL LWVI AND/OR ITS SUPPLIERS BE LIABLE FOR ANY DIRECT, INDIRECT, PUNITIVE, INCIDENTAL, SPECIAL, OR CONSEQUENTIAL DAMAGES ARISING OUT OF OR IN ANY WAY CONNECTED WITH THE USE OF OWSA OR WITH THE DELAY OR INABILITY TO USE OWSA, OR FOR ANY INFORMATION, PRODUCTS, AND SERVICES OBTAINED THROUGH OWSA, OR OTHERWISE ARISING OUT OF THE USE OF OWSA, WHETHER BASED ON CONTRACT, TORT, STRICT LIABILITY, OR OTHERWISE, EVEN IF LWVI OR ANY OF ITS SUPPLIERS HAS BEEN ADVISED OF THE POSSIBILITY OF DAMAGES.

Miscellaneous
The Terms will be governed by and construed under the laws of the State of Florida, excluding only its conflict of law provisions. Each party to OWSA Terms hereby submits to the exclusive jurisdiction and venue of the courts within the City of Tampa, County of Hillsborough, State of Florida, U.S.A. and waives any jurisdictional venue or inconvenient forum objections to such courts. Any failure by LWVI to enforce any of its rights under the Terms or applicable laws shall not constitute a waiver of such right.

Privacy Policy
LWVI does not sell or rent nonpublic personal information about our members or former members. LWVI does not share non-public personal information about our members or former members with companies outside of LWVI, except where required or permitted by law. LWVI restricts access to your nonpublic information to its employees who need such information in order to provide products or services to you or to keep your account in good order. In compliance with federal regulations, LWVI maintains physical, electronic and procedural safeguards designed to guard and prevent misuse of your nonpublic personal information.

Internet Privacy Policy
LWVI will utilize any information collected to better understand its user's needs and provide better service. If any personally identificable information has been volunteered, LWVI may utilize that information to notify users about updates to OWSA and/or contact users for marketing purposes. The information that is personally identifiable will not be shared with or sold to any other organization for commercial purposes. Below is a list of non-volunteered information that will be collected automatically when visiting OWSA:
